Jason Arismendy is a New York State licensed Physical Therapist. He completed his physical therapy education from Columbia University with advanced training in orthopedics in 2010. His passion for helping others is the primary reason for pursuing a career in physical therapy. Jason has been active his whole life and has seen various orthopedic injuries first hand. These experiences made him interested in the field of physical therapy and sports rehabilitation. Since beginning at Forest Hills Rehabilitation, Jason has taken pleasure in returning dozens of patients to a restored level of function, as well as providing the foundation to prevent future injuries. Jason plans to continue his education and become an Orthopedic Certified Specialist.

Jim is a New York State Registered Physician Assistant ("P.A."), who is National Certified to practice medicine with his attending Physicians. Jim graduated from the Physician Assistant program at SUNY @ Stony Book in 1979 and has held numerous Physician Assistant and Supervisory positions in the field of medicine. Jim suffers from multiple spinal issues which lead him into his current field of Complementary and Rehabilitative medicine. Having personally benefited from the therapies, which included the combination of medical, chiropractic and physical therapy treatments, he was able to improve his quality of life. This is why Jim is able to empathize with his patients and assist them on a path to wellness without having to rely on addictive drugs or other potentially harmful medications. Do not let back pain or joint pain stop you from enjoying life. There are alternatives and I demonstrate that each day at work.

Regina Derewlana is an Ace certified Personal Trainer. She graduated from Nursing School and Physical Education College. Additionally she completed additional cep, mat pilates, and functional training courses to help enrich her education and experience in physical therapy and sports medicine. Along with close communication with doctors and physical therapists, she supervises, implements and adjusts rehab exercises to help our patients get optimal benefits from their physical therapy program. Regina is dedicated to helping our patients get the most from their physical therapy. She is also available for post rehab personal training to provide a safe, goal oriented and injury free exercise program.

With many years of education and valuable experience under her belt, Dr. Amato has treated many conditions ranging from acute and chronic pain: sports related overuse injuries, headaches, low back pain, and neck pain as well as herniated discs. Aside from being a licensed chiropractor, Dr. Amato is Board Certified in Sports Chiropractic through the American Chiropractic Association and is certified in Active Release Technique.
A cum laude graduate of Life Chiropractic College West in San Francisco, Dr. Amato is knowledgeable in various forms of holistic health care. She has even traveled to El Salvador in a humanities effort to provide chiropractic care to others and plans to return annually.
Dr. Amato has spent her whole life involved in various sports as an athlete and coach, mainly gymnastics, softball, and volleyball. This developed into a career working with athletes toward reaching their peak performance abilities. Aside from in office treatment, Dr. Amato works on athletes before and after tournaments and events on site. In July 2009, she had the opportunity to provide chiropractic care to professional volleyball players on the AVP tour.
Dr. Amato's goal is to help others restore and uphold their wellness, and educate them on adopting a healthier well-being through regular spinal adjustments and exercise.

Dr. Berns has been a Chiropractor for over 18 years. He holds a Bachelors Degree in Biology from the State University of NY at Albany and a Doctorate of Chiropractic from New York Chiropractic College. Dr. Berns was inspired to become a Chiropractor after getting injured at a summer job while in college. He went to a Chiropractor and was so impressed by how it helped him. He has been an advocate of Chiropractic ever since. He has never forgotten that Chiropractor who took such great care of him and that is what he strives to do with each and every patient he treats. Dr. Berns has treated patients of all ages, from 1 to 95. His patients have had positive results and he has educated many people on the benefits of Chiropractic. Every patient that he helps to have a better quality of life reminds him of why he became a Chiropractor.
Dr. Berns is also a Professor at both Nassau and Suffolk Community Colleges where he teaches Anatomy and Physiology.

Upon completion of a four year bachelor's program at the University at Buffalo, Dr. Timothy Barber went on to pursue his chiropractic education at New York Chiropractic College in Seneca Falls, NY.
Inspired by his own success as a chiropractic patient, Dr. Barber graduated with honors in April of 1997. Following an eight month associateship, Dr. Barber opened his flagship practice in Forest Hills, NY, where he continues to help patients of all ages, ethnicities, and socioeconomic backgrounds.
Dr. Barber's Statement:
"My practice philosophy has always been to deliver personalized care for each patient, with clearly defined goals and outcome assessments. Nutrition, physical fitness, and healthy lifestyle choices have long been a part of my personal life, and have always been at the forefront of my professional practice.
I am proud to say that I have never compromised patient care. From the very beginning I have employed every tool at my disposal to take patients from pain or sickness to wellness and beyond. Dr. Hammoud is a board-certified and licensed New York State physician. His practice focus is in urgent care and sports medicine. Before joining Forest Hills Rehab, Dr. Hammoud was previously an urgent care physician at Columbia University. He has also attended post-graduate conferences in sports medicine at New York University. He combines his clinical experience along with cutting edge medical treatments to help patients recover thoroughly from their injuries.
Dr. Hammoud is a strong proponent of complementary care. He witnesses the benefits patients receive when unique medical treatments are combined with physical therapy and chiropractic care.
